ACFS Reporting Form

Hello everyone,

The ACVO TASS Task Force has completed a reporting form for veterinary ophthalmologists experiencing cases of Anterior Chamber Fibrinoid Syndrome. This is the place to report patients who develop strands of fibrin within the capsular bag or spanning the anterior chamber within the first month after phacoemulsification. The form was based upon the ASCRS TASS reporting form. The form is long and very detailed. You can stop and save the form to complete submission later if you need more time, need to look up information, or need to verify information with other members of your staff.
